2012 Summer Season Adult Kickball League Team Registration

This Team Form must be received before deadline in order to be considered. The deadline for Summer 2012 is 5pm, May 18. All forms must be in the PARD Office before that time. On-line form will not be accepted unless the minimum of 10 players are listed on the form. Partial teams will not be accepted.

In addition to the Team Form, the Team Fee of $600 must be receipted on or before 5pm, May 18.

With enough teams in the league, would you prefer to play in a Competitive or Recreational Sub-Division?

In the event that more than twelve (12) teams register for a division, the division may be split into two sub-divisions.
a. Competitive Division –
i. Made up of teams that wish to play at a higher, more competitive level; and
ii. Teams that have a majority of players that finished 1st, 2nd or 3rd in the same Division (Women’s or Co-E) during any of the previous three seasons (Winter, Spring or Summer)
b. Recreational Division –
i. Made up of teams that wish to play at a recreational level; and
ii. Teams that are newly formed and/or that the majority of the players have not finished 1st, 2nd or 3rd in the same Division (Women’s or Co-E) during any of the previous three seasons (Winter, Spring or Summer)
c. Number of teams in each sub-division will be determined by PARD Office.

I agree that I am 18 years old or above, or the legal guardian of any child under the age of 18 years old participating in this program or event. I agree that falsification of any information on this information/registration form may disqualify me or my child from this program or event.
In consideration of the acceptance of my or my child's information/registration form for this program or event, I hereby release the CITY OF KYLE, KYLE PARKS and RECREATION DEPARTMENT, and their agents, employees, officers, and servants from any and all damages and injuries which may occur while I or my child participates in this program or event. I understand that the CITY OF KYLE PARD or program/event staff have the authority to remove participants from the program/event without refund.
